If you live within our practice area you may register by completing a standard new patient registration form and providing proof of identity – photo ID and proof of address – for example Passport, Birth Certificate, Driving Licence, utilities bill. Once registered your records will transfer from your current GP practice. You are required to complete a New Patient Health Questionnaire and the details from this will be recorded on your records.
A copy of the practice leaflet is available here.
Practice Area
We have a defined practice boundary area. Please contact reception to ascertain whether your address is within this area.

Named Accountable GP
When you register with the practice you will be given a named accountable GP who is responsible for your overall care at the practice. This does not prevent you from seeing any GP at the practice that you wish. You should contact the practice if you wish to know who this is, and if you have a preference as to which GP that is, we will make reasonable efforts to accommodate this request.
Armed Forces
Since 1 April 2015, GMS Regulations allow for a serving member of the armed forces of the Crown, who has received written authorisation from Defence Medical Services (DMS), to register with a GP in whose practice area they live or work during the period in respect of which that written authorisation is given, for a period of up to two years.
In these circumstances, DMS will retain responsibility for meeting occupational health needs, but the individual’s primary care needs would be delivered through registration for NHS primary medical services with a GP practice.
Veterans
When you leave the Armed Forces, you will need to register with your local GP. You should make sure that the GP has all the information they need about your current health, including any conditions related to your time in Service. This will help your GP to give you the right treatment and, if necessary, refer you to specific services for those who have been in the military. Please see here for more information.
What our patients say about us
“The medical staff are led by Dr Pickavance who is very professional, caring and extremely efficient. Nurse Lisa and her support Lisa are always accessible and offer really supportive advice. Similarly the administrative team are always really helpful and understanding of patient needs. I particularly commend the appointment system which allows for planned visits. However, perhaps the most excellent facility is the daily walk in clinic. This arrangement ensures that patients will be seen promptly when they are ill. This is a fantastic facility which is very much appreciated, particularly when compared to the experiences of my friends and family.”
“We are confident that the service offered is the best on offer. All doctors are supportive and our family doctor is extremely well liked. Very helpful and listens.”
“Friendly, professional approach by every member of the practice. The provision of morning open surgery is a bonus for someone who works permanent nights.”
“I have been attending this surgery for 25/30 years. I have never had cause to complain re any aspect of the surgery, ie reception or doctors – I have always been treated fairly and with respect.”
“The commitment of all of the staff, doctors and receptionists alike. The courtesy, efficiency and patience of everyone.”
“Friendly treatment. Prompt attention. Clean and attractive premises.”
“This is the best doctors I’ve been to in my life, even the receptionists are very good and polite. It’s made my day!”
“What I like is that I can see my own doctor at the walk in clinic, excellent service at reception and with all surgery staff.”
“We always get first class treatment. And are made to feel like a person and not a number. I would not hesitate to recommend any one to this surgery.”
